Clinical and biochemical evaluation of low protein diet in chronic renal failure in India

A clinical trial of a newly devised low protein Indian uremic diet was conducted in patients with chronic renal failure. The standard diet contained 30 g protein and 1800 cal; both vegetarian and non-vegetarian diets were planned. An exchange list of various foodstuffs was provided to the patients to break the monotony of fixed diet menus. Patients (25) with chronic renal failure of moderate severity, who had either a creatinine clearance of less than 10 ml/min, or a blood urea of more than 150 mg per cent with gastrointestinal symptoms, were studied. The patients were admitted to the hospital for an initial period of 4 wk for evaluation of their clinical and biochemical status and initiation of the new diet. The follow-up period varied from 6-104 wk. The results of the diet therapy were evaluated in terms of the clinical response, biochemical effects and change in performance capacity. Good response was characterized by lessening or disappearance of uremic symptoms like anorexia, nausea, vomiting and drowsiness, by fall in level of blood urea and by improvement in work capacity. Of the patients, 15 showed good response to diet therapy, 6 deteriorated after an initial good response and 4 had poor response to the treatment. Nutritional therapy probably has an important place in the conservative management of chronic renal failure.
