The case is presented of a 43-yr-old man with granulomata of the lung due to Cryptococcus neoformans. The tissue reaction consisted of typical tuberculoid granulomata with caseation necrosis. Organisms, observed in giant cells and necrotic tissue, measured 2.6 mu X 2 mu and were mucicarmine negative. Because of these findings an erroneous diagnosis of Histoplasma capsulatum infection was made. Subsequently, cultures of non-encapsulated C. neoformans were obtained. Removal of 2 nodular lesions in the left lung took place and no further therapy other than for min. temp. elevations was given. The patient remained asymptomatic (for 6 months to date) with complete disappearance of pulmonary lesions.
