Acute kala-azar with haemorrhagic retinopathy

A case of acute Leishmania donovani infection in a girl in Bengal, India, presenting with a daily double peak in temperature and acute chest symptoms, including dyspnoea and haemoptysis, is described. The girl had severe anaemia and retinal haemorrhages in both eyes which resulted in permanent impairment of vision. No eosinophils were seen in the peripheral blood. The patient responded well to intravenous sodium antimony gluconate therapy and the eosinophil count returned to normal.
