Selective vitamin B12 malabsorption in two siblings

A brother and sister, both 2 years old, showed megaloblastic anaemia fully responsive to parenteral vitamin B-12. When seen, their serum vitamin B-12 values were 70 and 60 pg/ml. Gastric juice from both contained acid and functional intrinsic factor. Both showed proteinuria, which cleared after injection of vitamin B-12. The serum contained transcobalamin II but no antibodies to intrinsic factor. Schilling tests showed vitamin B-12 malabsorption uncorrected by pig intrinsic factor or pancreatic extract.Both patients are maintained on monthly injections of cyanocobalamin and are haematologically normal.
