Three children with the Imerslund-Grasbeck syndrome are presented. This familial disorder consists of megaloblastic anemia due to selective malabsorption of vitamin B12 and benign proteinuria. In 2 of the children the anemia remitted on parenteral treatment with vitamin B12 while the proteinuria was unchanged. In the 3rd child, the syndrome was suspected because of the family history and diagnosed before onset of the anemia. This is the 1st reported case of the Imerslund-Grasbeck syndrome where the selective vitamin B12 malabsorption was diagnosed before the megaloblastic anemia was evident.
