Isonicotinic acid hydrazide and pyridoxine deficiency

Six adults with pulmonary tuberculosis who were receiving 1 g streptomycin and 300 mg isonicotinic acid hydrazide daily were given a methionine loading test to estimate vitamin B-6 status. They were then given 50 to 100 mg pyridoxine daily by mouth or parenterally for 15 to 30 days, in addition to the antitubercular drugs. The loading test was repeated after 1 month on pyridoxine. All 6 patients developed signs of peripheral neuropathy during treatment with isonicotinic acid hydrazide; after loading with methionine they excreted more cystathionine in urine and the cystathionine:cysteine sulphinic acid ratio increased. With pyridoxine they showed clinical improvement. Biochemical abnormalities indicated that cystathionase activity is low in vitamin B-6 deficiency induced by isonicotinic acid hydrazide.
