A case is presented of an aorto-gastric fistula secondary to disruption of the distal anastomosis of an abdominal aortic graft. The original graft was placed between the diaphragmatic portion of the aorta and the aorta just aboye the superior mesenteric artery take-off. The anastomatic disruption led to formation of a pseudoaneurysm which then spontaneously ruptured into the posterior wall of the stomach. The mode of presentation with multiple gastrointestinal bleeding episodes and the need for a high index of suspicion in making the diagnosis are stressed. Alternatives in operative management are considered.
