The transmission, pathogenesis, dissemination, diagnosis and treatment of Cryptosporidium infections in man are reviewed, with particular reference to immunosuppression. Cryptosporidium is responsible for only 2-5% of acute enteric conditions. Mortality in immunosuppressed and AIDS patients due to diarrhoea is nearly 80%. No effective treatment is available currently; most forms of therapy (spiramycin, erythromycin and somatostatin) result in only a reduction in the amount of daily stool volume. The most promising is Zidovudine, which seems to reduce both the stool volume and to clear oocysts from the body.
