A case of fatal disseminated Strongyloides stercoralis infection is reported in a 29-year-old man 8 months after kidney transplantation. Retrospectively, occasional eosinophilia could have been the only alerting sign after transplantation. Subsequently, 3 other transplant patients with strongyloidiasis were diagnosed and treated successfully, suspected on the basis of persistent eosinophilia.
