Fifty one Rh sensitized mothers and their neonates were studied. A correlation is made of zonar espectrophotometry according to Liley's schema, with indirect bilirrubin and hemoglobin of the newborn. The indirect bilirrubin levels in umblical cord blood are higher and they tend to raise more rapidly, as well as lower Hb with tendency to decrease faster in children from mothers with zones II and III espectrophotometry. A graphic of exchange transfusions done in new borns, is presented; exchange transfusion was done in all the cases, except one of zone I. Immediate exchange transfusion (before one hour) should be done in all new borns from mothers with zones II and III espectrophotometry,
