A gastric ulcer in juxtapyloric position had developed in a 70-years old woman, who had been an steroids and ACTH for years. This ulcer perforated into the first part of the duodenum, leaving a bridge of tissue covered with mucosa (septum) between the perforation channel and the pylorus, and leading thus to the formation of a "double plyorus".
