Vaginal candidiasis: a source for fungal peritonitis in peritoneal dialysis?

A case is reported in a 47-year-old diabetic woman from Washington DC, USA, who was undergoing chronic peritoneal dialysis (CPD) for end-stage renal failure, who developed abdominal pain and cloudy dialysate in 1997, 4 months after antibiotic therapy for pneumonia. One week earlier the patient had noted a white vaginal discharge associated with pruritus. Antibiotic therapy was unsuccessful. Cultures of the peritoneal dialysate fluid grew Candida albicans. The patient was successfully treated by immediate catheter removal and oral fluconazole and flucytosine.
