Thoracic outlet syndrome is a common complication among patients with cervical strain injury referred to a general neurologic practice. Thirty-six percent of the cases seen with cervical strains in the current series of 111 patients developed this complication. Correct diagnosis will allow for improved treatment of these patient and will serve to avoid unnecessary diagnostic procedures and ineffective and sometimes dangerous surgery such as cervical fusion. Thoracic outlet syndrome will continue to suffer under-diagnosis or mis-diagnosis unless there is a broader acceptance of the current clinical criteria for establishing the diagnosis. At the present time there remains no universally accepted laboratory test, either electrodiagnostic or vascular, to confirm the diagnosis in the majority of cases.
