A case of gastric tuberculosis is reported indicating the diagnosis difficulties. Though rare, such a disease can be seen even nowadays in our industrialized countries among non migrants. Laparotomy is almost always needed for diagnosis. Findings are described fully. Multiple fistulae, marked enlargment of the lymph nodes, ulcerations, should lead to the right diagnosis during the operation. This would be very important to avoid too large operations like gastrectomy thinking that one has to cope with peptic ulcere or a cancerl Furthermore the possibilities of medical treatment are very important and surgery should be as limited as possible. Sometimes the disease starts as an emergency by bleeding or perforation which enhances its surgical interest.
