Pneumocystis carinii pneumonia in an HIV-negative, 8-month-old infant

A case is reported in an 8-month-old male infant from the USA, who presented with respiratory failure. Chest radiography revealed bilateral diffuse alveolar disease with consolidation and air bronchograms. Silver stain of bronchoalveolar lavage specimens demonstrated P. carinii and cotrimoxazole was administered. Testing for HIV was negative but immunological evaluation revealed a humoral immunodeficiency characteristic of hyper-IgM syndrome.
