Thymoma, Pneumocystis carinii pneumonia, and AIDS

A case is reported in a 48-year-old man: a chest film revealed bilateral diffuse interstitial infiltrates and a mediastinal mass approximately 6 cm in diameter. The patient was admitted to hospital and given trimethoprim and sulfamethoxazole for possible Pneumocystis pneumonia. Biopsy of the mass on day 3 of hospitalization gave results consistent with spindle cell thymoma. Silver stains of lung tissue for Pneumocystis were positive. The patient had a strongly positive titre for HIV on ELISA. The patient deteriorated, and despite aggressive resuscitation and administration of pentamidine, he died on the 7th day of hospitalization. This is considered to be the first reported case of thymoma in a patient with AIDS.
