The WHO recommendations for the general management of patients with malaria are given. These include the procedures to be adopted in diagnosis, the management of the unconscious patient, and the recognition of complications in severe malaria. In areas with significant quinine resistance, tetracycline (250 mg 4 times a day) is recommended and should be given for 7 days. Treatment of chloroquine-resistant malaria with intravenous quinine (20 mg/kg over 4 hours followed by 10 mg/kg over 4 hours and repeated every 8-12 hours until oral medication can be given) or quinidine gluconate (15 mg/kg over 4 hours followed by 7.5 mg/kg over 4 hours and repeated until oral medication can be given) is described. The recognition and management of patients with cerebral malaria is discussed.
