A personal account is given of experiences of working with village health workers in Orissa State, eastern India. A methodology is described that uses the skills, knowledge and efforts of the villagers themselves to improve their health conditions. Such an approach requires the health staff to have not only a basic attitude of confidence in people and their abilities, but also a capacity to look critically at their own outlook and prejudices. The experience shows that this type of strategy can give these communities real hope for the future in achieving 'health for all'.
