Fluconazole treatment of persistent Cryptococcus neoformans prostatic infection in AIDS

A total of 14 patients with AIDS who had completed standard amphotericin B therapy with or without flucytosine for Cryptococcus neoformans meningitis were treated for a median of 30 wks with fluconazole for persistent prostatic cryptococcosis. Seven of the 14 patients responded to fluconazole after a median of 4 wks of treatment (range 1-27 wks). The response rate was higher with higher doses and the one patient who received 600 mg/d had sterilization of multiple large prostatic abscesses. Dissemination with recurrent meningitis occurred in 2 patients after a total of 113 months of observation. Both cases had ongoing cryptococcuria but negative serum and cerebrospinal fluid cryptococcal antigen titres. It is concluded that fluconazole at a dose of 200-600 mg/d should be used to treat persistent prostatic cryptococcosis.
