A case in a 28-yr-old man is reported. The patient presented with pyrexia and headaches and mental confusion. India ink preparation of cerebrospinal fluid revealed encapsulated yeast cells, and C. neoformans, subsequently identified as biovar gattii, was isolated. Despite itraconazole therapy, the patient died. The isolation of this particular biovar is thought to be the first to be reported from a patient with acquired immune deficiency syndrome (AIDS) and also the first documented isolation of this variety in Central Africa in 17 yr.
