In moderate renal failure, where histological abnormalities are less severe, calcitriol decreases the bone turnover by decreasing the extent of surfaces covered by osteoid especially of woven texture, the number of bone cells, the resorption surface and endosteal fibrosis when present; an improvement in mineralization status also occurs. In advanced renal failure calcitriol is effective in reversing secondary hyperparathyroidism. There are inconsistent data regarding the effects of calcitriol on osteomalacic abnormalities. However, most studies report a decrease in osteoid volume, fibrosis and the number of osteoclasts and osteoblasts.
