Extrapulmonary pneumocystosis in patients taking aerosolised pentamidine

Common features in patients in whom extrapulmonary Pneumocystis carinii infection developed following use of aerosolized pentamidine have been identified by review of 10 cases reported between April and October 1989. These included widely disseminated infection often without concurrent pneumonia; long duration of AIDS; prolonged use of aerosolized prophylaxis; and concurrent treatment with zidovudine. It is suggested that use of zidovudine and aerosolized pentamidine prophylaxis while reducing early morbidity and mortality in HIV infection, may allow for the emergence of more insidious subacute disorders such as extrapulmonary pneumocystosis.
