Localized nerve damage recorded intraoperatively in carpal tunnel syndrome

Electrophysiological intraoperative evaluation of the median nerve at the wrist was done in 14 patients with clinical and neurophysiological signs of carpal tunnel syndrome (CTS). Sensory neurophysiological data (SNAP latency, duration and amplitude) were recorded from the middle finger by stimulation proximal (S1), at the center (S2) and distal (S3) to the carpal ligament, in 14 CTS and 5 control patients. Sensory conduction velocity (SCV) did not allow localization of conduction abnormalities at a single level in the CTS. In 4 out of 14 of our patients the slowing was found in the proximal segment S1-S2. In 8 out of 14 conduction abnormality was found in both the S1-S2 and S2-S3 segments. In only one patient a distal (S2-S3) slowing was found.
