Nutritional and metabolic modulation of the carcinogen nitrosodimethylamine in chronic renal failure

Eighteen patients 23 to 65 years old with end-stage renal failure and on maintenance haemodialysis were deprived of food for 12 h before intubation. Initial baseline samples of gastric and duodenal aspirate were obtained before 8 patients were given 6 g ascorbic acid daily or 4 non-absorbable antibiotics (4 patients); 6 patients were given 25 g ethanol in water or ginger ale. Ingestion of ascorbic acid had a definite inhibitory effect on generation of N-nitrosodimethylamine (NDMA) in the stomach but a questionable effect in the duodenum. There was a reduction in NDMA values in 3 patients after administration of antibiotics. Blood NDMA values increased significantly in all patients from baseline concentrations at 5 and 10 min after drinking ethanol.
