Phonatory disorders in Parkinson disease. An unusual case

A patient with Parkinson's disease had great difficulty in speaking spontaneously or to command, and presented on motor speech examination alterations of the phonatory mechanisms typical of extrapyramidal diseases. The surprising feature is that he spoke normally when he dreamed or automatically answered the telephone. The disability, recently reported in progressive supranuclear palsy, has been called apraxia of phonation, but rather that a distinct entity, it may be considered an expression of a general akinesia of extrapyramidal disorders.
