Aspergillosis of the brain: report of an autopsy proven case

A case is reported in a 16-yr-old girl who presented with fever, joint pain, generalized skin rash, renal failure, pleuritis and pericarditis. Systemic vasculitis was diagnosed and corticosteroid therapy was given. Renal function stabilized and the skin rash improved, but pyothorax developed; antibiotics were also administered. Fifty days after admission the patient's condition deteriorated and a CT scan of the head revealed multiple brain abscesses. Operative drainage was performed and culture of pus from an abscess grew Aspergillus fumigatus. Amphotericin B and 5-fluorocytosine were given, but the patient died 3.5 months after diagnosis of aspergillosis.
