Non-enzymatic glycosylation of plasma proteins in the evaluation of blood glucose balance in diabetics: determination of fructosamine

To assess the effectiveness of plasma fructosamine determination as a short-term index of metabolic compensation in diabetic patients, we studied 12 diabetics and 8 healthy controls. In diabetic patients, blood glucose (three times: h 8, 12, 18), total cholesterol, triglycerides, glycosylated haemoglobin and fructosamine were measured every ten days for a period of 20 days. At time 0, fructosamine resulted to be significantly higher in diabetics than in controls (3.75 .+-. 0.54 mmol/l, M .+-. SD vs 2.26 .+-. 0.14, p < 0.001). A positive correlation was found, in diabetic patients, between glycemia at time 0 and fructosamine at time 10 (p < 0.005), as well as between glycemia at time 10 and fructosamine at time 20 (p < 0.001). Also, a positive correlation was observed between glycosylated haemoglobin and fructosamine at time 0 (p < 0.05). Our data show that fructosamine determination may represent an effective marker of short-term glycemia balance (15-20 days) in diabetic patients in respect to the glycosilated haemoglobin, which is a well known long-term index (3 months) of metabolic compensation.
