Immunohistochemical detection of carcinoembryonic antigen in cholangiocarcinoma and intrahepatic stones

From 1983 to March 1986, we examined 10 cases of cholangiocarcinoma (C.C.) coexisting with intrahepatic stones (IHS), 7 of the well differentiated type and 3 of the anaplastic type. In the same period, 13 cases of IHS were studied for comparison. All the specimens were stained with PAP method for carcinoembryonic antigen (CEA). All the specimens taken from the cases of IHS were negative for CEA staining except one focus in one case which showed atypical epithelial change. All 7 cases of well-differentiated C.C. were positive for CEA staining. The other 3 cases with anaplastic C.C. showed negative for CEA staining. It is suggested that atypical changes induced by IHS play a role in the pathogenesis of cholangiocarcinoma. Continuous long-term follow up of the cases of IHS to clarify the significance of positive CEA in the specimen of intrahepatic bile duct is necessary.
