The pure erythrocytosis cases reported in the literature, with regard to those characterized by plasmatic elevated erythropoietin, are reviewed. The case of a woman, affected with elevated erythropoietin familiar pure erythrocytosis discovered following the onset of Budd-Chiari syndrome ascites, is subsequently described. Some characteristics of this observation, unusual in the pure erythrocytosis cases, are discussed. This case is included in the 5-6 group of Kulkarni's classification concerning the primary erythrocytosis syndromes.
