The authors presents his experience of cross face nerve transplant. The scanning electron microscopic study of three grafts removed at the time of reoperation for clinical failure provides certain conclusions confirming the clinical impressions. 1) The definite validity of the procedure, as re-innervation actually occurs. 2) The inadequacy of the procedure for the restoration of elevation of the oral commissures. On the basis of these findings, the author prefers double nerve transplant (Miehlke) in indications for hemi-facial re-innervation. The results of double-grafts cannot be analysed because of the small number of cases and the insufficient follow-up.
