Detection of intraventricular blood in pure subarachnoid hemorrhage. Physiopathologic significance

We have studied the CT findings of 12 cases of pure subarachnoid hemorrhage without associate intraparenquimatous hematoma. A relation was found between the detection of intraventricular blood and any degree of ventricular dilatation. As justification of this feature, we suggest a ventricular reflux mechanism, quite similar to the metrizamide and isotope cisternography studies in normal pressure hydrocephalus patients. This hydrocephalus is produced by a villi arachnoid blockage caused by red blood cells in subarachnoid space.
