In 50 patients with acute coronary failure the incidence, type, diagnostic and prognostic significance were assessed of the electrocardiographic changes of the atrial complex. The index of Morris was found to be the most sensitive electrocardiographic indicator of changes in the left atrium in these cases. Completion of myocardial infarction in patients with acute coronary failure was frequently preceded by electrocardiographic signs of left atrial strain. Abnormal shape and/or presence of pathological values of the indices of the atrial syndrome in ECG taken on the day of admission to hosptial were without any prognostic importance, but the persistence of these changes or their worsenning in acute coronary failure suggested increased risk of infarction development.
