Pathogenesis of cardiogenic shock

Acute experiments were conducted on 34 adult dogs to study the consequences of ligating the coronary artery of the heart under conditions of fentanyl-thiopental anesthesia and artificial lung ventilation. In 14 dogs the manipulation induced a reaction which was marked by a clearly defined phase course and the development of fatal shock with a characteristic torpidity syndrome. Primary cardiogenic shock developed in 6 animals immediately after occlusion of the coronary artery and they died within a few minutes. Cardiogenic shock terminated in the period of stabilization in 5 dogs and they survived. Shock did not develop in 9 dogs who were subjected to a false operation. Comparison of the changes of the main hemodynamic values in the first 3 experimental groups provides evidence that the reduced heart output is a consequence of the development of cardiogenic shock.
