Metastatic pulmonary and mediastinal carcinoma from testicular teratocarcinoma differentiating into a completely mature teratoma--a case report

A 34 year-old man underwent a left inguinal orchiectomy for a teratocarcinoma of the testicle in June, 1978. He was well until February 1984, when chest X-ray film revealed abnormal shadows in the right lung field and mediastinum. Transbronchial lung biopsy specimen of the right lung tumor revealed to be a metastatic lesion from the testicular teratocarcinoma. He was treated with three courses of cisplatin, vinblastin and bleomycin, and the radiotherapy with no change in tumor size in the right lung and mediastinum by chest x-ray. In June 1984, right lower lobectomy and mediastinal lymph node dissection was performed. Pathological examination of the extirpated tumors disclosed completely benign teratoma. He has no recurrence of the disease in 8 months' follow up. We conclude that an aggressive surgical intervention for the metastatic testicular carcinoma is important in order to determine the precise pathological category of the residual tumors and necessity for additional chemo- and radiotherapy.
