Primary pulmonary sporotrichosis occurring in two meat packers

Patient 1, a 57-yr-old white man who had worked at the meat packing plant for 13 yr, presented with spontaneous pneumothorax. Following reexpansion of the lung, chest X-ray revealed extensive cavitary disease of both upper lobes with an air-fluid level of the left apex. The patient had a 5-month history of a productive cough with fever, myalgias and night sweats. He was readmitted 6 months later due to persistence of symptoms. Bronchial washings and sputum grew Sporothrix schenckii. Treatment with amphotericin B (total dose 3.5 g) was successful. Patient 2, a 52-yr-old white man, who worked at the same packing plant, complained of shortness of breath for 3 yr prior to hospitalization. Three weeks before admission he noted blood streaked sputum and fever. Chest X-ray disclosed pleural thickening and apical cavitary infiltrate. Sputum cultures grew S. schenckii. Treatment with amphotericin B (total dose 1.6 g) was successful.
