The authors describe several modifications to Silber's operation of testicular autotransplant. They disect and section the epigastric vessels at an higher level, and the spermatic vessels at a lower level, making thus possible to do the whole operation through a simple inguinotomy. They clamp the end of the sectioned spermatic cord so that blood vessels, through colateral circulation, may fill up: this renders more visible the testicular artery, so hard to locate in normal circumstances due to its small diameter and lack of pulsation. Finally, the authors follow a different technique to the one described by Silber when carrying out the arterial anastomosis; in their opinion, this makes suturing easier. These modifications have been done on 5 of the 6 patients they have operated on, and they claim excellent results.
