Since 1989, the correspondents have conducted a programme of assisted conception. After more than 1000 insemination attempts in 350 couples and nearly 200 pregnancies there was not a single case of female seroconversion or paediatric infection. These results therefore suggest that it may be important to control for the main variable in sexual transmission-the presence of HIV in semen. It is recommended that every effort should be made to reduce the risk of sexual transmission of HIV in HIV-discordant couples who want to have a child. Reproductive counselling should emphasise the risk of infection associated with each instance of unprotected intercourse.
