In a prospective study of local anesthesia of the great toe 61 patients undergoing 70 procedures of either avulsion of the great toenail or avulsion plus an ablative procedure were randomised into two groups. Group A received local anaesthetic medially and laterally to the neurovascular bundle only; Group B received in addition an injection of local anaesthetic to the dorsum of the toe. Adequate anaesthesia was produced only where local anaesthetic had been injected medially, laterally and dorsally but was often found to be inadequate if the dorsal injection was omitted.
