During the periods from August of 1977 to Decemember of 1984, total of 364 patients who had received aorto-coronary bypass surgery were treated at our ICU, the surgical results and the incidence of major complications occurred in the early postoperative periods were analysed. The overall mortality rate was 4.1%, though that of 251 patients in last 3 years was improved to 1.2%. The incidences of major complications were listed as follows; arrythmia was 41.4% low cardiac output syndrome (LDS) was 20.9%, perioperative myocardial infarction was 9.9% respiratory failure was 8.5% and coronary arterial spasm was 2.2%. The incidence of LDS which showed very high in previous period was markedly decreased to 6.0% in latest 3 years. The management of early postoperative care were discussed in this paper especially refering to a view to prevent and treat these major complications.
