Differential diagnosis between amoebic liver abscess and acute cholecystitis using a discriminant coefficient

Two hundred cases of acute cholecystitis and 100 cases of amoebic liver abscess were studied in order to identify features useful for differential diagnosis. Using discriminant analysis, 6 variables were selected: hepatomegaly (present in amoebiasis), Murphy's sign (absent), previous abdominal pain or indigestion (absent), history of dysentery (present), duration of pain ( 48 h) and facial pallor (present). Using the discriminant coefficient of the 6 signs and symptoms, patients with amoebic liver abscess and acute cholecystitis could be differentiated with an accuracy of 96%. When applied to 20 new cases of acute cholecystitis and 21 of amoebic liver abscess, the discriminant coefficient diagnosed correctly all but one case of amoebic liver abscess.
