Reversible acute interstitial nephritis associated with indomethacin

Nonsteroidal, antiinflammatory drugs were implicated in causing acute renal failure of several distinct patterns. [Antiinflamatory drugs inhibit prostaglandin synthesis. The major mechanism of renal function impairment induced by these drugs is the reduction of renal blood flow due to the loss of the vasodilatory activity of prostaglandins in states of kidney ischemia.] A case of reversible acute renal failure associated with indomethacin therapy was reported. Renal function recovered upon cessation of indomethacin treatment. Renal biopsy disclosed the presence of tubulointerstitial nephritis which was thought to be responsible for the acute renal failure in this patient. Following presentation of this unusual side effect within the wide spectrum of renal and electrolyte disorders related to nonsteroidal, antiinflammatory agents, it was recommended that kidney function indices should be frequently tested in patients receiving indomethacin.
