We have studied the fractional excretion of sodium at the time of admission in 11 patients with drug-induced acute tubulointerstitial nephritis and acute renal failure. Urinary sodium excretion was low in 5 cases and high in the remaining 6. There was no difference between both groups of patients regarding the degree of damage of the renal interstitium; by contrast, patients with higher sodium excretion had a more severe tubular degeneration. In this group of patients the impairment of the renal function was more severe and the prognosis was worse.
