The reported clinical analysis was carried out on 21 patients dying after rupture of the free wall of the left ventricle during acute myocardial infarction. Rupture of this part of the heart wall accounted for about 23% of all deaths in acute myocardial infarction. Most ruptures occurred on the 1st day and between days 6th and 8th. The course of myocardial infarction up to the time of rupture development was without significant complications. In about 30% of all patients symptoms of "subacute" rupture of this wall were observed. It is stressed that in these cases the slit of the rupture was only half as long as in the remaining cases.
