Cases (1705) of acute myocardial infarction were observed, with 320 lethal outcomes (18.8%). In the group of lethal outcomes in 71 cases (22.2%), the cause of death was rupture of the free wall of the left ventricle. The clinical characteristic features in the group of patients dying from this rupture and in the group dying from other causes were compared. Analysis by the method of chi square showed that the observed patients with this rupture had no more frequent incidence of anterior wall infarction and were not older than the remaining patients. The observed rise in the incidence of this rupture could not be explained by earlier hospitalization of these patients. In 40.5% of cases death caused by this rupture occurred within the first 24 h after the onset of symptoms of acute myocardial infarction.
