Reconstruction of the pavilion of the ear in microtia

Auricular reconstruction for microtia in 2 stage is proposed. In the 1st, auricular cartilaginous framework is carved with the 6th and 7th rib cartilages that form the antihelix and the helix, respectively; this cartilaginous skeleton is then introduced under the mastoid skin. As a 2nd stage, the cartilaginous skeleton and the skin are separated from the cephalic ground and a skin graft is applied in the area. This technique has been used for 5 yr and applied to 18 patients and the obtained results, even though they had been analyzed in a subjective way, were considered good in comparison to the other results. Comparing this technique to those proposed during the last 25 yr, it is considered simple and recommended because it decreases the number of surgical stages to reach the final result and abolishes the transfixant stiches and utilizes just 2 rib cartilages. Consequently there is a decrease in hospitalization time, exemption of risks of pressure ulcers formation along the new cartilaginous framework and less surgical trauma.
