Comparison of hypothermia and normothermia methods of cardiopulmonary bypass: hemodynamic and hormonal responses

The effect of hypothermia and normothermia methods of cardiopulmonary bypass (CPB) on hemodynamics and hormonal responses was studied in 35 patients who underwent coronary artery bypass surgery. Hypothermia method of cardiopulmonary bypass (HCPB) induced higher responses in systemic vasular resistance (SVR), mean arterial pressure (MAP) and core and toe temperature than in normothermia method in cardiopulmonary bypass (NCPB) during bypass. HCPB produced higher SVR and lower cardiac index (CI) and core temperature than NCPB after the operation. HCPB induced lower plasma levels of cortisol and insulin compared with NCPB during bypass. HCPB did not affect the plasma concentrations of ADH and ACTH. HCPB caused vasoconstriction and deteriorated hemodynamics during bypass and during early post operative period. HCPB influenced the hormonal response during CPB. Evidently there is little rationale for the routine use of systemic hypothermia during coronary surgery.
