Hormonal and hemodynamic responses during and after coronary artery bypass surgery in diabetic patients

The hormonal and hemodynamic responses in diabetic and in non-diabetic patients were studied during and after coronary artery bypass surgery. The plasma concentrations of insulin was lower in diabetic patients compared with that of non-diabetic patients during cardiopulmonary bypass. No significant difference in plasma levels of cortisol and glucagon was detected. A lower left ventricular systolic work index and a higher systemic vascular resistance in diabetic patients were detected compared with those in non-diabetic patients in perioperative periods. The present authors' findings suggest that the diabetes mellitus deteriorates the insulin secretion and causes the impairment of cardiovascular function during and after coronary artery bypass surgery. Therefore, the anesthetic management of coronary artery bypass surgery in diabetic patients require the adequate hormonal and cardiovascular consideration.
