Coccidioidomycosis tenosynovitis: case report and review of the literature

A 46-yr-old man from Canada visited Arizona in 1977 and 1 yr later developed leukaemia. After chemotherapy he became febrile and pulmonary Coccidioides immitis infection was diagnosed. Amphotericin B was given but a culture of a scalene lymph node remained positive. In 1982 he developed pain and swelling in the left arm and the pulmonary coccidioidomycosis recurred. Amphotericin B was again given but the arm pain continued. Tenosynovectomy was performed and pathology of the excised tissue revealed tenosynovitis due to C. immitis. Ketoconazole was given and led to reolution of the infection.
