Tubal sterilization is a factor in 5-10% of cases of ectopic pregnancy while the rate for all sterilization failure is 15-30%. 2 cases of postligation ectopic pregnancy are reported. In both cases pregnancy occurred within 2-2.5 years of ligation. In the 1st case it occurred following hysterectomy and ligation and in the 2nd following puerperal ligation. For both women laparotomy was performed and the left tube was religated. It has been found that ectopic pregnancy happened 5 times more often in the tube distal to the site of sterilization than proximal to it, and those cases happened due to fistula on the proximal site and transmigration of sperm. To prevent the latter an extra silk ligature on the proximal tube in Pomeroy's technique has been used. Other modifications of Pomeroy's technique might also be necessary.
