Pirenzepine in gastric and duodenal ulcer: randomized comparison with cimetidine

Two groups of 10 patients each, formed on the basis of a pre-established scheme of randomization were treated with pirenzepine (PRZ 100 mg/day) and cimetidine (CMT 1 g/day), respectively. Basal gastric acid secretion and secretion after histamine stimulation, basal gastrinemia after protein stimulation, and endoscopic findings were assessed before starting treatment and after 6 wk of therapy. PRZ had definite inhibitory action on parietal cells and possibly on antral G cells as well since after blockage of the HCl-gastrin feedback no hypergastrinemia was observed. This multiple action accounts for the global endoscopic findings which showed healing or improvement in 90% of the patients treated with PRZ against 70% of the ones treated with CMT. In spite of an antisecretory action approximately identical to that of PRZ, in some cases CMT requires longer supportive therapy with antacids and brings about an unbalance of HCl and gastrin which, failing other compensatory effects, causes increases of gastrinemia and possible hyperacid-hypersecretory rebounds.
